STEP

The Bicultural Patient Advocacy and Breast Cancer Services Project IV - Howard County Health Department
The grant will offer breast cancer screening and follow-up support to Hispanic, Korean, and other ethnic minority women in Howard County who have limited or no health care coverage. The grant will fund two patient advocates - one Hispanic and one Korean - who speak English and either Spanish or Korean fluently and who are experienced with the Hispanic or Korean cultures. It will also fund contractual interpreters for women of various ethnic groups in Howard County.

Breast Cancer Screening, Diagnosis, Treatment, Case Management, and Office Support for Minority Women - Baltimore County Health Department
This grant will continue to provide breast cancer screening to three minority populations receiving culturally appropriate services to Baltimore County Department of Health. Hispanic, Korean, and African American women will receive breast cancer screening, diagnosis, and treatment. A Korean and a Latino outreach worker will provide culturally appropriate intervention during this process.

Closing the Gaps: Hispanic Women and Breast Cancer - Wicomico County Health Department
The grant will provide a Hispanic interpreter to identify, refer and enroll underinsured and uninsured Hispanic women in Wicomico and Somerset Counties into a breast cancer screening program. The interpreter will also provide case management services such as interpretation/translation and transportation services.

Breast Friends - Lifebridge Health System
This program is designed to support, educate and advocate for the unique needs of young survivors. The program will provide monthly professionally facilitated support groups where young survivors can share their experiences and learn about specific issues such as fertility and sexuality concerns, dating relationships, and caring for young children. The program will develop an advocacy and awareness campaign to educate the medical and legislative communities about the special issues and needs of this underserved group of cancer survivors.

Cancer Awareness, Resources, Exams, Screening (CARES) - Chase Brexton Health Services, Inc.
The CARES (Cancer Awareness, Resources, Exams, & Screening) for Women project incorporates three main objectives. The first is providing direct access to referrals and follow-up for screenings through patient navigation services. The second is medical provider education. The third objective is establish a system for ensuring women who benefit from the services of CARES are re-screened at least once every three years.

Bit Zdorovim - Jewish Family Services of Central MD
This program is designed to overcome cultural obstacles to breast care for these Russian-speaking immigrant women. This support includes assistance in obtaining referrals, making appointments, addressing their fears and concerns, escorting them to and from any procedure they may need and translating information between physician, client and family.

Just Us - Cecil County Health Department
The Just US Women Outreach Project representing church communities and other interested partners in Cecil, Harford and Kent Counties will provide screening services to women less than 40 who are at high risk for breast cancer and establish the need for Meet and Talk Long Term Survivor Groups in each county. The project will continue the training established in the past year of Just US! Outreach Teams; educating women about the importance of breast cancer screening, identifying resources in their communities and referring high risk women less than 40 years of age meeting eligibility criteria to receive breast cancer screenings and follow up services.

STEP's to Breast Health on the Mid-Shore - Shore Regional Cancer Center
The objective of STEPs is to improve breast health care services to African-American, Hispanic, and medically underserved women in the Mid-Shore region of Maryland (Talbot, Dorchester, Queen Anne, and Caroline counties). Services include: breast cancer screening, diagnosis, and treatment; transportation and translation services; case management of the target populations; and ongoing education and outreach to referring physicians and community leaders about the services.

Assisting Un-Insured Women in Anne Arundel County Access Mammography Services - People's Community Health Centers
The goal of this project is to increase mammography rates in underserved women. The targeted population will be women over the age of 40 who are uninsured, or underserved as a result of racial or cultural differences, and who seek primary care at People's Community Health Centers, Incorporated.

Mammograms for All - Carroll County Health Department
Grant funds will provide a mammogram and clinical breast exam to those who cannot enroll in the Breast and Cervical Cancer Early Detection Program because it is full. Special efforts will be provided for outreach to educate and arrange for services, along with an allowance for monies to be used if treatment is necessary. Translator services and transportation for the Hispanic community will be available as needed.


CRAFT

Enhancing Accrual of Women with Breast Cancer to NCI-Sponsored Clinical Trials - Johns Hopkins University, School of Medicine, Sidney Kimmel Comprehensive Cancer Center
This grant will provide for a clinical trials research nurse to assist local surgeons, medical oncologists, and radiation oncologists to enroll patients to NCI sponsored clinical trials for breast cancer.

 

Increasing Physician Participation and Patient Accrual to NCI Sponsored Breast Cancer Clinical Trials - Shore Health System
This project will focus on continuing to accrue patients to NCI-sponsored clinical trials for breast cancer treatment and prevention. This will be done through the continuing work of a clinical trials research nurse, who will educate physicians, members of the community, and potential candidates about clinical trials and the potential benefits of participation. The nurse will also assist in the enrollment process and in data management. The emphasis will remain on increasing the enrollment of all eligible women, but with a continuing focus on educating members of the African American community about clinical research and the need for involvement.

MAKE A DIFFERENCE

Washington County Hospital New Collaborative Make a Difference Program - John R. Marsh Cancer Center
This grant will continue to provide breast cancer awareness education, free clinical exams in convenient rural and town settings, transportation to a free mammogram if indicated, and follow-up case management and treatment as necessary. This new project, aiding 320 women, will increase the number of screening clinics, at times more accessible to the working poor in a wider variety of locations. Nurse practitioner use will increase, and translation services will be available for non-English speaking patients. Finally, the new project will include the expansion of the Outreach Worker role to include a Navigator component for women with suspicious or positive findings, connecting them to where they need to be and to the resources available.
