Komen Maryland License Plates

Komen Maryland License Plates can be requested by sending an e-mail with your address to: licenseplates@komenmd.org

1. Give us your address so we can mail a form to you. You will need to fill out that form and mail it back to us at Komen Maryland.
2. When returning the form, include two checks. Make the first one payable to the MVA for $25 and the second check made payable to Komen Maryland for $25. (This is the check that goes directly to Komen Maryland!)
3. Please allow 2-4 weeks for your Komen Maryland license plates to arrive. You will receive two plates with new stickers. After you receive your new license plates, you can either mail or hand-deliver your old license plates to your nearest MVA location.
